Lesson: Complete the form, “My Profile (LCECHS)”

Follow the following instructions to create your narrative document:

  • Click here to access the profile form
  • Make a copy of the document and click onto the box 'Share it with the same people' (this will ensure that I have access to the document that you have created.
  • Replace 'Copy of' in the name of the document with your first and last name (properly capitalized)
  • Select 'SHARE' to confirm that the document has been shared to cpc@collegeplanningcohort.com
  • You will need a copy of your high school transcript, and SAT/ACT Score Reports, to complete the 'Academic Profile.'
  • Academic Profile
    • Enter a fraction for Class Rank, e.g., 10/160
    • Enter your highest SAT and ACT Scores for each section
    • Enter total Honors, AP, and College Classes
    • Enter ONLY AP and College Classes in the table, together with grades, AP exam scores, and college credit hours
    • Enter any academic awards or clubs
  • High School Classes Taken and Grades Earned
    • Enter any middle school classes taken for high school credit
    • Enter all classes taken throughout high school
    • Enter level of class, grade earn, and high school credit (e.g., .5 or 1)
    • Enter number of credit hours earned for college classes
  • Extracurricular Activities, Employment, and Awards
    • Enter each job or activity in which you participated during each year of high school
    • Enter any award related to your activities received during each year of high school
    • Enter the date that you began the activity and the ending date (enter present for the ending date of any activity in which you are currently involved)
  • Leadership Profile
    • Enter all leadership roles throughout high school
    • Place an * (asterisk) in front of any leadership roles that you plan to assume during your senior year
  • Career and College Major: You may complete this area now or wait until you complete the 'Careers and College Majors' activity
